Group Adult Foster Care Program (GAFCP)
Group Adult Foster Care (GAFC) is a Medicaid-funded long-term care community option that provides elderly and/or disabled clients, known as members, with daily personal care assistance, meals, housekeeping, and personal emergency response services.
Each member’s care is overseen by a team comprising an RN, care manager, and home health aide. They work together to ensure members can live healthy and safe at home and avoid institutionalization.
Vine Management is an experienced Group Adult Foster Care provider and has been delivering these services for more than 10 years. Our Group Adult Foster Care program is licensed by MassHealth, and the program is free to those who qualify. It is only available through licensed providers such as Vine.
